理工学術院の学生が新型コロナウィルス感染症やインフルエンザに罹患した場合は、以下を確認のうえ、申請フォームにより理工学術院事務所におしらせください。また、申請フォーム入力後、大学から連絡する可能性がありますので、早稲田アドレスを定期的にご確認ください。
If FSE students get infected for COVID-19 or influenza, please follow the procedures first and then notify the FSE Office by using the following application form. Also, please check your Waseda e-mail regularly, as the university may contact you after you fill out the application form.

1.大学への報告/How to report the university
申請フォームから連絡してください。
Please contact the university by submitting the application form.
※医療機関・保健所発行の診断書や治癒証明書がある場合は、申請時に添付してください。診断書の代替として陽性者登録センター登録時の返信メッセージのスクリーンショットなどを証明書類として添付可能です。
*If you have a medical certificate or any documents (issued by a medical institution or health center) to prove that you have been cured, please attach them for the application form. You can attach such as a screenshot of reply message you received from the Positive Persons Registration Center to the application form as certification documents in the stead of a medical certificate.
2.授業欠席について/Absence from class
授業履修については「出席停止」措置となりますが、無症状あるいは軽症等の理由により、医師が特別に認め、ご自身でも可能と判断した場合に限り、オンライン授業の履修ができます。授業を欠席する必要性が生じた場合は、WasedaMoodleの「Message My Teacher」などを通じて、科目担当教員に事情をご説明ください（対面授業の場合は、治癒後、教室でご相談いただくことも可能です）。
You will be suspended from attending classes. However, if you are asymptomatic or have a minor illness, you may take online classes only if your doctor specifically allows it and you are able to do so. If you need to be absent from classes due to this reason, please explain your situation to the instructor of each course via “Message My Teacher” on WasedaMoodle etc. (In the case of face-to-face classes, you can also consult with the instructor in the classroom after healing).

※51号館理工統合事務センター教学支援課カウンターで手続きのうえ、ご自身で教員に書類のコピーを提出して申し出る必要があります。
