Absences from Classes Due to Infectious Diseases (e.g., Influenza)

If you are unable to attend classes due to an infectious disease which is classified as “Infectious Diseases Prevention in School”, you may request special consideration by completing the necessary procedures at the SILS office. Please first check whether the disease you contracted falls under this category.

Please refer to the information available on the Health Support Center Web page for details regarding preventing the spread of infectious diseases on campus.
Infectious Diseases Prevention in School
*Please note that infectious diseases not listed on the above page are not eligible for the “Notification of Absence due to Quarantine for Infectious Diseases”.
Procedure
- If you contract an infectious disease, notify the SILS/GSICCS office using the following online form.
【SILS/GSICCS】 Report of confirmed infectious diseases - Once you have received permission to attend school, or the “suspension period of coming to school” has ended, ask your physician to fill out the designated Certificate of Recovery from Infectious Disease Form.
Certificate of Permission to Attend School - Fill out and submit the “Agreement of Authorization” and “Notification of Absence” with the “Certificate of Recovery from Infectious Disease Form” to the SILS/GSICCS office.
- Present the Notification of Absence to your course instructors and request special consideration.
*Please contact the instructor of the class you miss by yourself. The SILS office does not contact instructors on your behalf regarding class absences.
*The final decision on a student’s absences will be left to the discretion of the instructor of each course.
*Regarding the COVID-19 and influenza (except for specified avian influenza), it is not required to submit “Certificate of Recovery from Infectious Disease” after the period of suspension from classes. Instead, you may submit proof of illness (medical certificate, etc.) that confirms the diagnosis and absence(quarantine) period along with a Notification of Absence document.
