学校において予防すべき感染症に罹患した場合
If You Contract an Infectious Disease to Be Prevented at School

「学校において予防すべき感染症」に分類される感染症に罹患した場合は、学校における感染拡大防止のため、学校保健安全法等に基づき、所定の期間は出席停止となります。対象となる感染症および出席停止期間については、以下の保健センターWebサイトを確認してください。

If you contract an infectious disease classified as an “infectious disease to be prevented at school,” you are required to suspend attendance for the designated period in accordance with the School Health and Safety Act and related regulations, in order to prevent the spread of infection on campus. Please check the Health Support Center website below for the relevant infectious diseases and suspension periods.

Students who have contracted any of the infectious diseases listed on the website above.

手続方法 / Procedures

1. 以下の申請フォームに必要事項を入力し、大学に報告してください。
   ※MyWasedaへのログインが必要です。

   *You must log in to MyWaseda.

2. 授業等を欠席する場合は、Waseda Moodleの「Message My Teacher」等により、各科目の担当教員へ事情を説明し、指示を仰いでください。

   If you need to be absent from classes, please explain your situation to the course instructor via “Message My Teacher” on Waseda Moodle or by other appropriate means, and follow their instructions.

3. 出席停止期間終了後、必要書類をIPS事務所に提出してください。
   ※新型コロナウイルス感染症および季節性インフルエンザについては、登校許可証明書の提出は不要です。感染症名および出席停止期間が確認できる診断書等で代用できます。

   After the suspension period has ended, please submit the required documents to the IPS Office.
   *For COVID-19 and seasonal influenza, the Certificate of Permission to Attend School is not required. A medical certificate or other document showing the name of the infectious disease and the suspension period may be submitted instead.
