FAQ

These are some of the frequently asked questions regarding the Waseda University Student Health Promotion Mutual Aid Association. Check the Overview of medical fee reimbursement and refer to the questions below.

Waseda University Student Health Promotion Mutual Aid Association

Your “Student ID” is your membership card for the Waseda University Student Health Promotion Mutual Aid Association. When you visit a contracted hospital, be sure to bring your Student ID and insurance card.

Medical Fee Reimbursement System

Please promptly go to the medical institution where you were examined to present your student ID card.
Application at the University office will not be accepted.

* Among the contracted medical institutions, “Waseda Skin Clinic” will accept only when the student’s student ID card is presented during the current month of the treatment. If you present your student ID card after the month following the month in which you received treatment, you will not be eligible for medical reimbursement even if you present your student ID card. Please be careful.

When sending a photo of your insurance card during the online medical procedure, please also send a photo of your student ID card.
Specifications may differ among medical institutions. If you are unsure about the transmission method, please inquire and confirm with the medical institution in advance by yourself.

Have the “Insurance Card for Separately Living Dependent” issued.

The “Insurance Card for Separately Living Dependent” shall be issued when you submit your “Certificate of Enrollment” or “Certificate of Residence” to the issuing organization of the insurance card of the supporter.

The “account under the student’s name” registered on MyWaseda must be an ordinary savings account at Japanese financial institutions in your name. (Be sure to avoid failure to register/incorrect information.)

There are especially many cases in which the name of the student and the name of the account holder differ. Be sure to check that the account is properly registered before applying.

For treatment covered by insurance received in clinics/dental offices and pharmacies, you can receive reimbursement regardless of the location of the hospital in Japan. All treatment received overseas shall be outside of the scope.

You can download the “Application” from the Waseda University Student Health Promotion Mutual Aid Association website and print it out.

Fill out the “Application” yourself and submit it by the application deadline along with the “receipt” (a copy is accepted) issued by the hospital.

However, all of the following contents must be included in the receipt.

  • Name of the patient
  • Date/year of the treatment
  • Name of the medical institute
  • Receipt seal of the medical institute
  • Insured medical fee points
  • Receipt amount

You may also apply by mail. Mail the “Reason for postal mailing form (Student ID copy must be affixed)” along with the “Application” and “Receipt (a copy is accepted)” by the application deadline.

*Be sure to mail the application in a traceable means, such as simplified registered mail, so that it arrives by the application deadline.

Note that we shall not return the receipt attached to the application.

Be sure to bring your Student ID to check your membership status. Without your Student ID, we shall not be able to accept the application.

Apply the reception counter by the 10th day of the third month after the treatment (the following weekday if the 10th day is a Saturday/Sunday/holiday or Summer/Winter Vacation of the university). Note that no application submitted after the deadline shall be accepted.

For treatment in a “contracted medical institute,” the reimbursement shall be transferred to the “account under the student’s name” registered on MyWaseda in the middle of the month two months after the visit month. (Example: Treatment in April -> Transfer in the middle of June)

For treatment in institutes other than “contracted medical institutes,” the reimbursement shall be transferred to the “account under the student’s name” registered on MyWaseda in the middle of the month following the visit month or two months after the visit month (*).

*Transfer shall be made in the middle of the following month for applications by the 10th and the middle of the month two months later for applications filed on the 11th and later. (Example: Applied on April 10 -> Transfer in the middle of May, Applied on April 11 -> Transfer in the middle of June)

Be sure to check the transfer record with account book entry at the bank.

It is possible that the “account under the student’s name” was not properly registered or updated. Check to make sure that the “account under the student’s name” is properly registered on MyWaseda first.

If you have medication dispensed at a pharmacy, fill out entry line (1) and entry line (2) of the “Application for Healthcare Benefit” yourself and submit it to the reception counter along with the receipt (a copy is accepted) issued at the pharmacy.

The application shall not be necessary if the dispensing was made by one of the contracted pharmacies near campus.

It has been determined that the treatment at an osteopathic clinic, the treatment for acupuncture/moxibustion/massage, and the therapeutic orthosis will be outside of the scope of the medical fee reimbursement system from April 1st, 2022. For more information, please click here.

Based on the past reimbursement record within the fiscal year, the reimbursement shall be made within the annual reimbursement limit of 60,000 yen.

Eligible period of medical fee reimbursement

For those who graduated in March of 2020

Applications can be accepted for treatment up to the end of March (March 31), 2020. The application deadline is June 10 (Wed), 2020. If you plan to apply for the graduation ceremony, make a copy of your Student ID before returning it.

For those who graduated in September of 2020

Applications can be accepted for treatment up to the end of September (September 30), 2020. The application deadline is December 10 (Thu), 2020. If you plan to apply for the graduation ceremony, make a copy of your Student ID before returning it.

Application method

In the case of contracted medical institutes (clinics/dental offices/pharmacies)

If you had presented your Student ID at the counter of the contracted medical institute, the contracted medical institute automatically links data. You don’t need to apply yourself.
*However, do not cancel the bank account registered as the “account under the student’s name.”

In the case of non-contracted medical institutes (clinics/dental offices/pharmacies)

Apply at a specified office counter or by mail.

*If you are postal mailing the application, include:

  1. Application for Healthcare Benefit
  2. Receipt (a copy is accepted)
  3. “Reason for postal mailing form (Copy of the Student ID must be affixed)”

During your “study abroad” time overseas, the Japanese Health Insurance Act is not applicable. Therefore, you cannot receive medical fee reimbursement from the Waseda University Student Health Promotion Mutual Aid Association or other subsidies. Due to this, your membership fee for the study-abroad period (by the month) shall be reimbursed upon application. (However, this is limited to official study-abroad programs of the university.)

(1) Application method

Fill out the required items in the “Waseda University Student Health Promotion Mutual Aid Association membership fee reimbursement application” (also available at the Waseda University Student Health Promotion Mutual Aid Association office) and submit it to the Waseda University Student Health Promotion Mutual Aid Association office along with required documents. Check Partial reimbursement of the membership fee for details.

(2) Application period

By the 10th day of the third month after your return to Japan.* Applications past the application deadline and applications before studying abroad shall not be accepted.

(Example) If you returned to Japan on September 15: The application deadline is December 10.

(3) Reimbursed amount

The reimbursed amount shall be the membership fee amount (1,500 yen per 6 months) in principle. The study-abroad period shall be calculated by the month. Only the study-abroad months satisfying the whole month shall be applicable.

Example: If the study-abroad period was from April 15 to September 15

-> 1,000 yen for the 4 months of May, June, July, and August are reimbursed

If you cannot come to the reception counter by the application deadline for some reason, such as hospitalization and recuperation close to your parents’ house, you can submit the “Application” by mail. However, applications that did not arrive before the application deadline shall not be accepted.

Note that the application deadline is NOT the postmark deadline.

The “Application for Healthcare Benefit” and the “Reason for postal mailing form (Student ID copy must be affixed)” can be downloaded from the Waseda University Student Health Promotion Mutual Aid Association website.

Be sure to attach the following items when applying by mail (be sure to use “registered mail”):

  1. “Application for Healthcare Benefit”
  2. Receipt (a copy is accepted)
  3. “Reason for postal mailing form (Copy of the Student ID must be affixed)”

*Without the “Reason for postal mailing form (Copy of the Student ID must be affixed),” the application shall not be acceptable.

Is influenza vaccination within the scope of medical fee reimbursement?

Vaccinations are not part of treatment covered by insurance (100% self coverage), so it is excluded from the scope of medical fee reimbursement by the Waseda University Student Health Promotion Mutual Aid Association.

Refer to this for treatment not covered by medical fee reimbursement.

The definition for the annual reimbursement limit of 60,000 yen is calculated based on the visit days within the fiscal year (Example: From April 1, 2020, to March 31, 2021, for FY2020).

For example, if you submitted an application in May of 2020 for the treatment received on March 31, 2020, the medical fee reimbursement will be counted as medical fee reimbursement for “FY2019” instead of “FY2020.”

You can apply as long as the visit month is before the “Application” application deadline. When doing so, submit the receipt at the time of payment (the receipt will say “unpaid balance,” etc.), receipt at the time of treatment, and the itemized receipt.

*If you only submit the receipt for the “unpaid balance,” medical fee reimbursement is not applicable.

Health checkup coverage

Yes. The reimbursement period of health checkup coverage is from April to March of the following year. You can receive a reimbursement of up to 3,000 yen for one checkup during the fiscal year.

However, this is limited to checkups received in a specified contracted hospital. Refer here for the application deadline.

Apply to the reception counter by the 10th day of the third month after the health checkup (the following weekday if the 10th day is a Saturday/Sunday/holiday or Summer/Winter Vacation of the university), and bring your Student ID. Note that no application submitted after the deadline shall be accepted.
