Members of the Waseda University Student Health Promotion Mutual Aid Association shall be as follows.
*Commissioned students, non-degree students, students of the Center for Japanese Language, exchange students, and special foreign trainees are not members.
*Treatment in overseas medical institutes shall be outside of the scope of the Medical Fee Reimbursement System by the Waseda University Student Health Promotion Mutual Aid Association.
Your “Student ID” is your “membership card” for the Waseda University Student Health Promotion Mutual Aid Association.
If you are taking procedures, such as seeking treatment from a contracted hospital/pharmacy and medical fee reimbursement application, or participating in projects by the Waseda University Student Health Promotion Mutual Aid Association, you must present your “Student ID,” which is the membership card for the Waseda University Student Health Promotion Mutual Aid Association. Be sure to carry it with you.
The membership fee for the Waseda University Student Health Promotion Mutual Aid Association is 3,000 yen per fiscal year. 1,500 yen is collected for 6 months when the tuition is paid (twice in June and October of every year).
If a member is specified by the university as a tuition reduction/exemption target as a student affected by a disaster, etc., the entire membership fee to be paid during the reduction/exemption period shall be exempt. (It is not necessary to apply with the Waseda University Student Health Promotion Mutual Aid Association for membership fee exemption.)
The membership fee for the study-abroad period (by the month) shall be reimbursed upon application.
*If your study abroad is completed online, etc. in Japan without leaving the country, this shall be excluded from the scope of reimbursement. You can use various medical fee reimbursement systems, etc.
Fill out the required items in the “Waseda University Student Health Promotion Mutual Aid Association membership fee reimbursement application (for study abroad),” attach a document (a copy is acceptable*) as the proof of the cause, and apply with the Waseda University Student Health Promotion Mutual Aid Association office.
*Documents (i) and (ii) below (a copy is acceptable) shall be required.
(i) “Letter of acceptance” issued by the hosting (study abroad destination) university or “Study abroad permission” issued by Waseda University
(ii) Document that can confirm the Japan departure date (Example: Passport page with the exit stamp, departure/return ticket (such as an e-ticket or boarding pass))
*You can also apply by postalmail(must arrive by the application deadline). Please mail things through traceable means, such as registered mail, to avoid failed delivery, etc. To mail the application, the “Reason for postal mailing form (Student ID copy must be affixed)” shall be additionally required.
You must file an application by the 10th day of the third month after your return to Japan.
(Example) If you returned to Japan on September 15: The application deadline is December 10.
*Applications past the deadline and applications before studying abroad shall not be accepted.
The reimbursed amount shall be the membership fee amount (1,500 yen per 6 months) in principle. The study-abroad period shall be calculated by the month. Only the study-abroad months satisfying the whole month shall be applicable.
Example: If the study-abroad period was from April 15 to September 15
1,000 yen for the 4 months of May, June, July, and August is reimbursed
Students who lost their student status due to expulsion, deletion, etc., in the middle of a semester (months other than March/September) shall be reimbursed with the membership fee for the remaining period (by the month) upon application.
*Only if the membership fee has been paid.
Fill out the required items in the “Waseda University Student Health Promotion Mutual Aid Association membership fee reimbursement application,” attach a document (a copy is acceptable) as the proof of the cause, and file the application with the Waseda University Student Health Promotion Mutual Aid Association office.

By the 10th day of the third month after the finalization of the cause.
*Applications past the deadline and applications before the university status change shall not be accepted.
The reimbursed amount shall be the membership fee amount (1,500 yen per 6 months) in principle. The remaining period shall be calculated by the month. Only the remaining months satisfying the whole month shall be applicable.
Example: If you left the school as of October 20 in the sophomore year (and you have already paid the membership fee from October of sophomore year to the end of March)
1,250 yen for the 5 months of November, December, January, February, and March is reimbursed
In case of any inconsistency between the Japanese version and the English version, the Japanese version shall prevail.
