The Waseda University Student Health Promotion Mutual Aid Association (established in 1964) is a mutual aid organization unique to Waseda University that promotes activities to maintain/improve the health of members and projects, such as medical expense provision and health checkup coverage, with the membership fees paid by the members (students).
It is not a public insurance union designated by the government, such as the National Health Insurance and health insurance unions.
Submit the application form to the reception counter by the 10th day of the third month after the treatment (or the following business day if the 10th day is a Saturday/Sunday/holiday or summer/winter break of the university). Note that no application submitted after the deadline shall be accepted.

Personal information protection
The Waseda University Student Health Promotion Mutual Aid Association shall only use personal information, such as diagnosis information, provided by members or contracted hospitals/pharmacies and managed by the Waseda University Student Health Promotion Mutual Aid Association for processing the applications for medical fee reimbursement from the Waseda University Student Health Promotion Mutual Aid Association and shall never use such information for other uses. The Waseda University Student Health Promotion Mutual Aid Association shall prevent the leakage, loss, or damage of personal information and take other necessary and appropriate measures to safely manage personal information.
In case of any inconsistency between the Japanese version and the English version, the Japanese version shall prevail.
