サークル等で陽性者等が発生した場合の報告方法について
サークル等で新型コロナウイルス陽性者・濃厚接触者が発生した場合は、下記の〈記入例〉に基づき、速やかに学生生活課([email protected])まで〈報告事項〉の内容を報告・提出してください。
なお、件名は「陽性者等の発生について(サークル名)」としてください。
〈報告事項〉※複数名の陽性者・濃厚接触者が発生している場合、2人目以降は⑤~⑩を報告してください。

⑫:7/30からサークル合宿を実施していたが、8/1に課外次郎が体調不良となり、東京へ帰宅するよう指示した。課外次郎を除くサークル員3名を濃厚接触相当者として隔離をしていたが、本日8/2に3名のうちの他大生が新たに体調不良となったため、合宿を中止とし、現在全員帰宅途中である。なお、帰宅途上において課外次郎より陽性であった旨の連絡があった。

なお、サークル員に新型コロナウイルス陽性者が発生した場合、サークル活動は、一旦、全サークル員について、停止となります。サークルからの報告内容および陽性者以外のサークル員の体調等を踏まえ、活動再開日等について指示します。濃厚接触者(「濃厚接触相当者」含む)はPCR検査を推奨します。
また、SNS等を含め、感染者情報を発信したり、感染者を誹謗中傷したりすることのないよう、全サークル員に徹底してください。
サークル内での感染拡大を防止するため、迅速な対応をお願いします。

How to Report the Occurrence of a Person Has Tested Positive for COVID-19 in a Circle, etc.
If a person has tested positive for COVID-19 or a person identified as a close contact in a circle, etc., please report and submit the contents of the [Matters to be reported] to the Student Affairs Section ([email protected]) as soon as possible based on the [Example of Entry] below.
The e-mail subject line should read “Report of COVID-19_Circle Name”.
〈Matters to be reported〉 *If more than one person have tested for COVID-19 or identified as close contacts, please report items (5) to (10) for the second and subsequent persons.

*A “list of contacts” must be attached because the person testing for COVID-19 has had contact with a circle member two days or more prior to the onset.

(12) Circle training camp had been held since 7/30, but on 8/1, KAGAI Jiro became ill and was instructed to return home to Tokyo. Three members of the circle, excluding Jiro, were quarantined as those who had close contact with the group, but today, 8/2, another student from other university became ill, so the camp was cancelled and all circle members are currently on their way home. On their way back home, they were informed that KAGAI Jiro has tested positive for COVID-19.

If a circle member tested positive for COVID-19, circle activities will be temporarily suspended for all circle members. Based on the reports from the circle and the physical condition of circle members other than the person tested positive for COVID-19, instructions will be given regarding the date of resumption of circle activities, etc. PCR testing is recommended for a close contact (including “equivalent to a close contact “).
In addition, please ensure that all circle members must not disseminate information on infected persons or slander infected persons, including on social networking services.
Please take prompt action to prevent the spread of infection within the circle.
