If you are visiting a “non-contracted hospital” or receiving medication from a “non-contracted pharmacy”

If you received treatment from a clinic/dental office other than contracted hospitals within Japan or if you received medication from pharmacies other than contracted pharmacies, fill out “Entry line (1)” and “Entry line (2)” on the “Application for Medical Fee Reimbursement” yourself and submit it along with the receipt (copy is accepted) at the Waseda University Student Health Promotion Mutual Aid Association counter. (You must present your Student ID)

The application deadline shall be the 10th day of the third month after the month following the visit month. Be very careful regarding the application deadline. (No application passed the application deadline shall be accepted.)

The location where the “Application” is available at all times/reception counter 

If you received treatment without bringing your “insurance card,” insurance will not be applicable. You will be required to pay the medical fee in full at the hospital. In principle, this shall also be excluded from the scope of medical fee reimbursement by the Waseda University Student Health Promotion Mutual Aid Association. If the hospital suggests that they would switch to treatment covered by insurance if you bring your insurance card on a later date, be sure to bring your “insurance card” by the due date and switch to treatment covered by insurance. After this, apply for medical fee reimbursement with the Waseda University Student Health Promotion Mutual Aid Association.

Precautions 

Fill out “Entry line (1)” and “Entry line (2)” on the “Medical Fee Reimbursement System application” and submit it along with the receipt (a copy is accepted) indicating the below information at the Waseda University Student Health Promotion Mutual Aid Association counter.

How to summarize the Medical Fee Reimbursement System application

Once a month, summarize the visits from the month (1st through the end of the month) and fill out the application. The application deadline shall be the 10th day of the third month after the visit month. Note that no application submitted past the deadline can be accepted.

*If you filed multiple applications for visits to the same medical institute within the same month,1,000 yen shall be deducted from each payment.

Receipt amount

All of the above shall be included. Note that the receipt submitted at the time of application cannot be returned.

[Important] It has been determined that the treatment at an osteopathic clinic, the treatment for acupuncture/moxibustion/massage, and the therapeutic orthosis will be outside of the scope of the medical fee reimbursement system from April 1st, 2022. For more information, please click here.
