はじめに｜Introduction

【日本語】

大学院情報生産システム研究科（IPS）では、IPS設置科目の不合格評価（F・Q）について、所定の期間内に「確認依頼」をIPS事務所へ提出することができます。

本制度は、シラバスに記載された成績評価方法および評価基準と照らし合わせた際に、成績評価に誤りがあると考えられる具体的かつ客観的な理由がある場合に、IPS事務所を通じて科目担当教員へ成績の再確認を依頼するための制度です。

不合格成績について、学生から科目担当教員へ直接問い合わせることはできません。必ず本制度に基づき、所定のフォームからIPS事務所へ確認依頼を提出してください。

The Graduate School of Information, Production and Systems (IPS) accepts requests for confirmation regarding failing grades (F or Q) for courses offered by IPS. Such requests must be submitted to the IPS Office within the designated application period.

This system allows students to request that a course instructor reconfirm a grade through the IPS Office when there are specific and objective grounds for believing that the grade may be incorrect in light of the grade evaluation methods and criteria stated in the syllabus.

Students must not contact course instructors directly regarding failing grades. Requests must be submitted to the IPS Office through the designated form in accordance with this system.

1．本制度の趣旨｜Purpose of This System

【日本語】

本制度は、シラバスに記載された成績評価方法および評価基準に照らして、成績評価に誤りがあると考えられる具体的かつ客観的な理由がある場合に、成績の再確認を依頼するものです。

科目担当教員は、提出された確認依頼の内容に基づき、成績評価に誤りがないかを確認します。確認結果は、原則としてIPS事務所から学生へ連絡します。

【English】

This system allows students to request reconfirmation of a grade when there are specific and objective grounds for believing that the grade may be incorrect in light of the grade evaluation methods and criteria stated in the syllabus.

The course instructor will review the request and confirm whether the grade was evaluated correctly. In principle, the IPS Office will notify the student of the result.

2．申請の対象｜Eligible Grades

【日本語】

申請の対象は、次のすべてを満たす成績に限ります。

  • 当該学期の成績発表において発表された成績であること
  • IPS設置科目の成績であること
  • 不合格評価（F・Q）であること

次の成績は、本制度の対象外です。

  • 当該学期より前の成績発表において発表された成績
  • IPS以外の学部・研究科等が設置する科目の成績
  • 合格評価となった成績

【English】

A grade is eligible under this system only if it meets all of the following conditions:

  • The grade was announced as part of the grade announcement for the relevant semester.
  • The grade is for a course offered by IPS.
  • The grade is a failing grade (F or Q).

The following grades are not covered by this system:

  • Grades announced before the relevant semester
  • Grades for courses offered by schools, graduate schools, or other academic units outside IPS
  • Passing grades

3．本制度の対象とならない問い合わせ｜Inquiries Not Covered by This System

本制度は、次のことを目的とするものではありません。

  • 就職、進学、修了、奨学金その他の個別事情に基づいて、成績の変更を求めること
  • 単に不合格となった理由の説明を求めること
  • 答案、採点内容または詳細な採点基準の開示を求めること
  • 成績評価について科目担当教員と交渉すること

【English】

This system is not intended for any of the following purposes:

  • Requesting a grade change based on personal circumstances, such as employment, further study, program completion, or scholarships
  • Merely requesting an explanation of why a failing grade was received
  • Requesting disclosure of examination answers, details of the grading process, or detailed grading criteria
  • Negotiating a grade evaluation with the course instructor

4．申請時に記載する内容｜Information Required in an Application

【日本語】

申請にあたっては、科目名、科目担当教員名その他の必要事項を正確に記載してください。

具体的な根拠が記載されていない申請や、就職、進学、修了、奨学金等の個別事情に基づく懇願が含まれている申請は受け付けません。これらの事情や要望は一切記載しないでください。

シラバスに記載された成績評価方法ごとに、次の事項を具体的に記載してください。

  • 出席状況
  • 試験の受験状況
  • レポートや課題の提出状況
  • その他、成績評価に関係する客観的な事実
  • シラバスに記載された成績評価方法および評価基準に照らして、成績評価に誤りがあると考える具体的かつ客観的な理由

「単位がないと修了できない」「就職や進学が決まっている」「奨学金の受給に影響する」などの個別事情は、成績評価に誤りがあると考える理由にはなりません。

【English】

Applicants must accurately provide the course title, the name of the course instructor, and all other required information.

Applications that do not provide specific grounds for the request, or that include personal appeals based on circumstances such as employment, further study, program completion, or scholarships, will not be accepted. Do not include any such circumstances or requests in the application.

For each grade evaluation method stated in the syllabus, applicants must provide specific information regarding the following:

  • Class attendance
  • Participation in examinations
  • Submission of reports and assignments
  • Other objective facts relevant to the grade evaluation
  • Specific and objective grounds for believing that the grade may be incorrect in light of the grade evaluation methods and criteria stated in the syllabus

Personal circumstances, such as needing credits to complete the program, having secured employment or admission to another program, or being concerned about the impact on a scholarship, do not constitute grounds for believing that a grade evaluation is incorrect.

5．申請を受け付けない場合｜Applications That Will Not Be Accepted

【日本語】

次のいずれかに該当する場合は、申請を受け付けることができません。

  • 試験欠席、レポートや課題の未提出、出席回数不足等、明らかに不合格となる理由がある場合
  • 成績評価に誤りがあると考える具体的かつ客観的な理由が記載されていない場合
  • 個別事情に基づいて成績の変更を求める記述が含まれている場合
  • 本制度と関係のない記述が含まれている場合
  • 対象科目、対象成績、申請期間その他の申請条件を満たしていない場合

【English】

An application will not be accepted if any of the following applies:

  • There is a clear reason for the failing grade, such as absence from an examination, failure to submit a report or assignment, or insufficient attendance.
  • The application does not state specific and objective grounds for believing that the grade evaluation may be incorrect.
  • The application includes a request for a grade change based on personal circumstances.
  • The application includes statements unrelated to this system.
  • The application does not meet the requirements concerning eligible courses, eligible grades, the application period, or other application conditions.

6．申請方法｜How to Apply

申請は、このページの「申請フォーム」からIPS事務所へ提出してください。

  • 申請は、1科目につき1件です。
  • 複数の科目について確認を依頼する場合は、科目ごとに申請してください。
  • 不合格成績について、学生から科目担当教員へ直接問い合わせることはできません。

【English】

Applications must be submitted to the IPS Office using the application form provided on this page.

  • Only one application may be submitted for each course.
  • To request confirmation for more than one course, submit a separate application for each course.
  • Students must not contact course instructors directly regarding failing grades.

7．申請期間｜Application Period

【日本語】

申請期間は、成績発表日を含む5営業日（IPS事務所開室日）以内です。

理由のいかんを問わず、申請期間終了後の申請は受け付けません。申請を希望する場合は、成績発表後、速やかにシラバスおよび自身の受験・提出状況等を確認してください。

【English】

Applications must be submitted within five IPS Office business days, including the date on which grades are announced.

Applications submitted after the application period will not be accepted under any circumstances. Students who wish to submit an application should promptly review the syllabus and confirm their examination attendance, assignment submission status, and other relevant information after grades are announced.

8．申請後の流れ｜Procedure After Submission

【日本語】

  1. 学生が、申請期間内に所定のフォームから確認依頼を提出します。
  2. IPS事務所が、対象科目、対象成績、申請期間および申請内容等を確認します。
  3. 申請を受け付けた場合、IPS事務所から科目担当教員へ成績評価の再確認を依頼します。
  4. 科目担当教員が、シラバスに記載した成績評価方法および評価基準に基づき、成績評価に誤りがないかを確認します。
  5. IPS事務所から、学生のWasedaメールアドレス宛てに確認結果を連絡します。

回答は、申請期間の終了日から1週間程度を目安とします。ただし、科目担当教員の出張その他の事情により、回答までに時間を要する場合があります。

【English】

  1. The student submits a request through the designated form within the application period.
  2. The IPS Office reviews the eligible course, grade, application period, and details of the application.
  3. If the application is accepted, the IPS Office asks the course instructor to reconfirm the grade evaluation.
  4. The course instructor confirms whether the grade evaluation was conducted correctly based on the grade evaluation methods and criteria stated in the syllabus.
  5. The IPS Office sends the result to the student’s Waseda Mail address.

Students should normally expect to receive a response approximately one week after the end of the application period. However, the response may take longer due to the course instructor’s business travel or other circumstances.

Inquiry Form Regarding Failing Grades for IPS Courses

The application form is scheduled to become available on March 1, 2027, the date of the Fall Semester 2026 grade announcement.
The application form is currently under preparation.
