Application by postalmail

What you need to submit the application

  1. Application for Healthcare Benefit (“Medical care itemized receipt” in case of an orthopedic clinic or acupuncture/moxibustion/massage clinic)
  2. Postal Application Form (*Copy of the Student ID (front only) must be affixed)
  3. Receipts issued by the medical institute (a copy is accepted)

*You may also send applications for multiple months in one mail. If you are sending them at one time, only one “Reason for postal mailing form” shall be required.

(Applications must always be divided by the visit month.)

* While applications are accepted by regular untraceable mail, there are risks of delivery failure/delayed delivery/loss. We shall bear no responsibility for such events. We also shall not inform you of the arrival, so we recommend that you send applications in a traceable method, such as simplified registered mail, specific recording, registered mail, and Letter Pack (Plus/Light).

*Applications submitted by mail that are not postmarked by the application deadline, for any reason, will be deemed invalid.

*Even if the application is postmarked by the deadline, it will not be accepted if the postage is insufficient or if no stamp is affixed. Please be sure to check that the postage is sufficient before mailing.

Precautions

  • Applications must be postmarked by the application deadline (applications not postmarked by the deadline will not be accepted).
  • We shall not inform you of the document arrival or issue a receipt.
  • If the postage is insufficient or if stamps are not affixed, the application will not be accepted, and it will be invalid. Please resolve any postage shortages and reapply by the application deadline.
  • Unless they are incomplete, the transfer shall be made in the middle of the month in the following month for applications that arrive by the 10th. However, for administrative reasons, applications postmarked by the 10th of the month in which the medical treatment was conducted (or the next business day if the 10th falls on a non-business day) will be processed and the payment will be transferred by mid-month two months later, provided there are no deficiencies.
  • Be careful not to have incomplete documents or incomplete form. (The application shall not be accepted in case of incompletion.)
    *There are many cases in which the Student ID copy is not affixed, “Reason for postal mailing form” is incomplete, etc. Please be careful.
  • Receipts, etc. shall not be returned for any reason. If you require the receipts, submit their copies.
    *Note that copies on both sides and those on which advertisements, etc. are printed on the backside shall not be accepted.
  • Also, check here when applying.
  • Health checkup coverage other than medical fee reimbursement, Waseda University Student Health Promotion Mutual Aid Association membership fee reimbursement application, etc. shall also be accepted by mail.
    The postmark date must be on or before the application deadline (applications with a postmark after the deadline will not be accepted).
  • In case of questions regarding the entry method, etc., inquire with us via telephone or email.
  • You can apply for health checkup coverage and partial reimbursement of the Waseda University Student Health Promotion Mutual Aid Association membership fee using the above method. (Be sure to include the Reason for the postal mailing form (Student ID (copy) must be affixed).)
