Medical Fee Reimbursement System

If you are visiting a “contracted hospital” or receiving medication from a “contracted pharmacy”

“Contracted Hospitals/contracted Pharmacies” refer to hospitals/pharmacies selected according to the selection criteria of the Waseda University Student Health Promotion Mutual Aid Association. Waseda University Student Health Promotion Mutual Aid Association has concluded partnership contracts with contracted hospitals, dental clinics, and pharmacies near the campus.

1) What to bring

  1. When you visit a contracted hospital/pharmacy, be sure to bring your “Student ID” and “Insurance card” every time. Also, bring them in case of continued treatment. (It is not necessary to bring the “itemization” form)
  2. You will be required to temporarily pay the medical fee on your visit, so be sure to bring money to cover the fee. On a later date, the amount from which 1,000 yen for self coverage amount has been deducted shall be transferred to the account under the student’s name.
  • Your “Student ID” is your “membership card” for the Waseda University Student Health Promotion Mutual Aid Association. When you visit a contracted hospital, you are required to present your “Student ID.”
    *Unless you present your Student ID, they will not recognize you as a Waseda University student. You may not be able to receive medical fee reimbursement.
  • Regardless of which hospital to use, you are required to bring your “insurance card” (a copy is not acceptable).
    *The “Insurance Card for Separately Living Dependent” shall be issued when you submit your “Certificate of Enrollment” or “Certificate of Residence” to the issuing organization of the insurance purchased by the supporter.

2) What to do at the medical institute counter

  1. Present “Student ID” and “insurance card”
    Please show your “Student ID” and “insurance card” at the reception counter. Inform them that you are a Waseda University student and that you would like medical fee reimbursement by the Waseda University Student Health Promotion Mutual Aid Association.
    (Unless you present your Student ID, the hospital will not recognize you as a Waseda University student. You may not be able to receive medical fee reimbursement.)
  2. Informing of changes
    In case of the changes below, be sure to inform us of the changes.
    –If your Student Number has changed due to a change in your school/major, shift from undergraduate to graduate school, etc.
    *If your Student Number remains the same, you may be excluded from the scope of medical fee reimbursement for the reason of student status change.

3) You do not need to take the procedure to apply for medical fee reimbursement at the counter

If you visited a contracted hospital/pharmacy, the treatment/dispensing data will be directly sent to the Waseda University Student Health Promotion Mutual Aid Association from the contracted hospital/pharmacy, which will be processed accordingly. Therefore, you do not need to apply for the medical fee reimbursement.

*However, if you were treated at a contracted hospital and received medication from a non-contracted pharmacy, you need to apply at the application reception counter of the university. Fill out the “Application for Medical Fee Reimbursement” on your own and submit it to the reception counter of the Waseda University Student Health Promotion Mutual Aid Association Office along with the receipt issued by the pharmacy (a copy is accepted). (Refer to “To apply for medical fee reimbursement” for details.)

The medical fee shall be reimbursed to the “Personal Account” in MyWaseda. Unless you are properly registered, we cannot reimburse you. Note that you will be regarded to have abandoned the reimbursement if the process comes back as an error in 3 consecutive months, resulting in an invalid process. Check the transfer record with account book entry, etc. If the transfer has not been made, check if your account has been properly registered on MyWaseda. Please also refer to this.

In case of any inconsistency between the Japanese version and the English version, the Japanese version shall prevail.
