For compensation during regular curriculum/school events/extracurricular activities in off-campus facilities, you must submit the “Notification Form for Activities held Off-Campus” (for faculty and staff)/<For Circle>Notification Form for Activities held Off-Campus (including camp/tour) to the Student Affairs Section by 7 days before the activity start date excluding closed days (excluding Saturdays/Sundays/holidays/vacations) (the deadline must be strictly followed) regardless of the travel distance or whether or not nights are spent away.

Swiftly describe the situation of the accident in the “(1)Student Compensation System(Compensation for Injury)Accident Report [for accidents other than traffic accidents]” and submit it to the Student Affairs Section within 30 days of the accident. If you are unsure about how to fill out the form or have questions regarding the procedure, please contact the Student Affairs Section.
*If circumstances such as recovery from injuries make it difficult to submit the form within 30 days of the accident, it may be submitted after that time. If you will submit it more than 31 days after the accident, please also submit “(2) Statement of Reasons for Delayed Accident Report”.
In case of an accident during commuting or during transportation between facilities, submit the “Student Compensation System(Compensation for Injury)Accident Report [for traffic accidents]”. Also submit the following documents.
Traffic accident certificate (original or photocopy)/copy of commuter ticket (for those commuting on train)/copy of license (in case of an accident involving a motorcycle/automobile)
*Depending on the activity, the applicable number of outpatient hospital visit days differs. Please refer to “Paid compensation types and amounts” for details.
In case of a serious accident, such as an accident involving death, a different procedure will be required. Please immediately contact the Student Affairs Section.

After completion of treatment, the following items (3) to (7) must be submitted. We shall determine the applicability of compensation and the compensation amount based on the submitted document details. We may also conduct additional investigations, site check, etc. depending on the details.

(5) Receipts issued by the medical institute (original or photocopy)
(6) Medical Certificate (original or photocopy)
If the compensation claim amount (amount received as the compensation) exceeds 100,000 yen or if you have physical impediment, submit the “(6) Medical Certificate (original or photocopy)” prepared by your doctor.
For information regarding the compensation claim amount, refer to “Paid compensation types and amounts” and “Calculation example for the number of treatment days/compensation claim amount.” The final compensation amount shall be determined by the university.
If the compensation claim amount (amount received as the compensation) is 100,000 yen or less and if you do not have physical impediment, the indemnitee (student himself/herself) must fill out the specified “(4) Treatment Report” in their own writing, seal it, and submit it along with the “(5) Receipts issued by the medical institute (original or photocopy)”. In this case, “(6) Medical Certificate (original or photocopy)” by the doctor shall not be necessary.
*If you are hospitalized, you must submit documents prepared by the hospital, etc. indicating the number of hospitalization days (receipt is also acceptable) in addition to the above documents.
(7) Other
Depending on the situation, we may ask you to submit documents in addition to the documents (1) through (6) above.
The Student Affairs Section shall check the above documents and calculate the compensation amount.
After the calculation is completed, the university shall ultimately finalize the compensation amount. The compensation shall be transferred to the “account under the student’s name” by the university in principle. It normally takes approximately 3 months before the payment is made. However, this shall not apply to cases in which we conduct investigations with the hospital and the police for a major accident or documents are incomplete, etc.
We may also conduct investigations, site check, etc. depending on the details.
You may also apply for medical fee reimbursement by the Waseda University Student Health Promotion Mutual Aid Association.
