Procedure flow

Registration Procedure

Only activities approved by sections shall be applicable for application.

If you wish to participate in an internship, practice teaching, or volunteer activities and wish to register with Student Compensation System (Liability Coverage), notify the section hosting the activities, such as the affiliated section or Career Center that is mainly hosting the activities. If the main hosting section is unclear, notify the affiliated section. You can apply for Student Compensation System (Liability Coverage) only if the section deems the participation in the activities to be part of educational activities.

Applicable period of Student Compensation System (Liability Coverage)

The applicable period of the system shall be the duration of the activities.
(For example, if the same person participates in internship and practice teaching both, the person is required to file an application for each. Even if internship is in the same category, the internship shall require separate application procedures if the duration of the activities or activity location differ.)

The university shall pay the insurance premium involved with Student Compensation System (Liability Coverage) registration.

*Applications are accepted on an academic year basis. For activities that cross over from one academic year to another, you will need to submit another application at the end of the academic year for the new academic year on or after April 1.

In case of an accident

Notification of the accident

In case of an accident, immediately (within 30 days after the accident) provide the details of the accident to [email protected] (Student Affairs Section email list) after filling out the required items in the Student Compensation System (Liability Coverage) Accident Report [PDF format][Japanese text only] and call the Student Affairs Section (03-3203-4349) to inform them of the following information.

1) Name
2) Age/date of birth
3) Affiliation
4) Student Number
5) Phone number
6) Name of activities
7) Location of activities
8) Details of activities
9) Accident time and date
10) Location of the accident
11) Details of the accident
12) Cause of the accident
13) Name/age of the injured person
14) Extent of the injury
15) Damaged object
16) Extent of the damage (estimated cost)
17) Other
*If possible, keep a record of the situation with photos, etc.

Negotiations, etc. with the victim shall be performed by the student themself. However, agents and insurance company shall offer consultation. Insurance claim procedures shall differ for different accidents within the scope. We shall individually inform you of the specific procedure after the submission of the Accident Report.

Payment

The Student Affairs Section shall check the Students Compensation System (Liability Coverage) Accident Report [PDF format][Japanese text only] and send it to the insurance company. The insurance company shall check the details of the claim, review the case, and pay the insurance money to the bank account on the form.

In case of any inconsistency between the Japanese version and the English version, the Japanese version shall prevail.
