For those of you who are scheduled to graduate in March of 2020 Precautions regarding applying for medical fee reimbursement
◆You shall be able to apply for reimbursement up to diagnoses up to March 31 (Tue), 2020. The submission due for diagnoses in March of 2020 shall be June 10 (Wed), 2020.
◆The Student ID shall be collected in exchange for the diploma.
Make a copy of your Student ID before it is collected!
◆Check your “account under the student’s name” in MyWaseda before graduation!
You will no longer be able to register/change anything after graduation.
◆Do not close your bank account until the transfer is completed.

■ If you are mailing the form as a registered mail (must arrive by the deadline)

■ If you are asking a proxy to submit the form
