Certificate Issuance (For Current Students)
Online application for current student
In principle, current students are requested to apply online using the system below for the issuance of various certificates (excluding the Health Certificate and Student Discount Certificates).
However, as Health Certificates and Student Discount Certificates cannot be issued at convenience stores (see below), please do not apply online for these certificates. Instead, submit your application at the IPS Office counter.

How to Use Certificate Issuance Service 【for Current Students】
- Certificate of Expected Graduation is only available for the 2nd year master students basically and who completed the course registration for the required graduation (completion) credits if it is his/her final semester. The issuing date starts from noon on June 1 in spring semester and November 10 in fall semester (If the date falls on a Sunday or national holiday, issuance will begin at 12:00 noon on the next business day.). Please note the following periods are exceptional and you can not issue the certificate (about 10 days each for Feb, May, August and October) . Please ask IPS office for more details.
- The issuing date of Health Certificate also starts from noon on June 1 in spring semester and November 10 in fall semester (If the date falls on a Sunday or national holiday, issuance will begin at 12:00 noon on the next business day.).
- We can not check the delivery status over the phone if you made a request online. Please use the ‘Contact form’, which can be accessed from the email sent to you after your online application

Graduates have different application procedures and locations depending on their degree and year of graduation. Please check below.

How to Use Certificate Issuance Service 【for Alumni】
*For those who have changed their family name, a family register showing the family name change or a pension book showing the maiden name is required.
*The certificate is issued under the student’s name at the time of enrollment (before the revision).

Issuance of Health Certificates and Student Discount Certificates (For Current Students)
As the Kitakyushu Campus does not have an automatic certificate issuing machine, Health Certificates and Student Discount Certificates are issued at the IPS Office counter.
Please download, print, and complete the appropriate application form below, and submit it to the IPS Office. After submission, the certificate will be issued and handed to you on the spot (please note that some waiting time may occur).

The Student Discount Certificate may also be applied for via the MyWaseda online application form instead of submitting a paper form. After your application has been processed, you will receive an email notification when it is ready for pickup. Please bring your Student ID card and visit the IPS Office to collect it.

*Students of the IPS Course, School of Fundamental Science and Engineering, who receive research supervision at the Kitakyushu Campus may also complete procedures for the Student Discount Certificate at the IPS Office. However, as issuance is not available on the same day, eligible students are requested to apply via MyWaseda as noted above.
For the Health Certificate, please contact the office at the Nishi-Waseda Campus directly.
≪Application by proxy≫
(Student Discount Certificate and Health Certificate Only)
To protect personal information, Waseda University issues certificates only upon application by the individual concerned. However, if the applicant is unable to apply due to unavoidable circumstances, an application by proxy will be accepted.
If someone other than the applicant applies on their behalf, the proxy must submit the following to the IPS Office together:
-Letter of Attorney (Download)
-Completed Application Form
(For the Student Discount Certificate, submission via the applicant’s MyWaseda online application form may be used instead of a paper form.)
For the Letter of Attorney:
The handwritten names and handwritten seal or signature of both the applicant and the proxy are required.
A copy of the applicant’s identification document must be attached.
