Guidelines When Students are Diagnosed with Designated Infectious Diseases (COVID-19, Influenza, etc.) *Updated May 8, 2023*

Dear Students of the Center for Japanese Language (CJL),

Please read the university policies for designated infectious diseases and take the necessary procedures if applicable to you.
*Non-CJL students should follow the instructions from their affiliated school office.

1.Status Reports

If you are diagnosed with designated infectious diseases, please submit the online Report Form below to the CJL office.

Select “Report of confirmed infectious diseases such as COVID-19, Influenza or others.” After filling out your information, press the “save” button to submit.
If you have any medical documents to submit (a medical certificate, Certificate of Recovery, etc.), please upload them on the Report Form.
*Medical documents are required when you apply for a “Special Consideration Request for Absence” mentioned below on “3. Contacting Teachers / ‘Special Consideration Request for Absence.'”

*No identification of “close contacts” will be made following the downgrading of the legal status of COVID-19 to “class 5.” (Effective from May 8, 2023.)

 

2.Class Attendance Suspension

If you are infected with a designated infectious disease, the following attendance policies are applied.

  • Face-to-face classes:
    Attendance is suspended during the suspension period.
  • Online classes:
    Attendance is suspended during the suspension period. However, students may attend online classes when asymptomatic or having minor illnesses and when a doctor and the students themselves agree.

3.  Contacting Teachers / “Special Consideration Request for Absence”

①As soon as you know you will be absent from a class, you should inform your teacher of this. You can message your teacher using the “Messages My Teacher” on Moodle.

②If you wish to request consideration for absences due to a designated infectious disease, submit a “Notification of Absence due to Quarantine for Infectious Diseases /Special Consideration Request for Absence due to Quarantine for Infectious Diseases” (link below) to the CJL office. (If you have not submitted any medical document to prove your infectious disease name and period, please attach one to the form.) By submitting the form, you can ask your course instructors for consideration for your missing coursework, such as class attendance (including that for on-demand courses), submission of assignments, and exams. Please note that the final decision on the consideration will be made by the instructor(s) and that it is not guaranteed.
