本プロジェクトは，文部科学研究補助金・基盤研究(A)「医療・介護の持続可能な制度設計へ向けて－多国間比較研究による行政データの利活用（22H00067）」，及び，厚生労働科学研究費補助金（政策科学総合研究事業（政策科学推進研究事業））「レセプトデータ等を用いた、長寿化を踏まえた医療費の構造の変化に影響を及ぼす要因分析等のための研究（政策変更を「自然実験」とする弾力性の推計に係る実証研究）（22AA1002）」を原資として実施する．本プロジェクトでは，高齢者のwell-beingに直結する医療と介護の持続可能な制度設計の在り方についての具体的な検討を行うため，米国National Bureau of Economic Researchと，日瑞の19大学による「MIRAI2.0」の両プロジェクトに参画し，今後十数年間に，超高齢社会に突入する高所得国11ヵ国を対象とする国際比較研究を行う．制度設計を検討する上で，価格・数量政策と需給のステークホルダーのインセンティブとの関連性を見極めることが重要な鍵となる．本研究では，各国が保有する大規模行政管理情報を比較可能なデータに再構築し，各国内での制度・政策の変更，及び，国や地域間での違いを自然実験と捉え，反実仮想な場面での高齢者やサービス提供者の行動変容を明らかにする．また，そうした行動変容と，高齢者のwell-beingや医療・介護費用との間のメカニズムを解明する．更に，政策立案やサービス提供の現場での実装を念頭に置いたアプリ開発を行い，多様なステークホルダー間での科学的根拠に基づく政策的な議論に貢献する道を模索する．

This project is funded by the Ministry of Education, Culture, Sports, Science and Technology Grant-in-Aid for Scientific Research (A) “Toward Sustainable Institutional Design for Healthcare and Long-term Care: Utilization of Administrative Data through International Comparative Research (22H00067)” and the Health and Labour Sciences Research Grant (Policy Science Comprehensive Research Project) “Research on Factor Analysis Affecting Changes in Healthcare Cost Structure Considering Longevity Using Receipt Data and Other Sources (Empirical Research on Elasticity Estimation Using Policy Changes as ‘Natural Experiments’) (22AA1002).”
This project conducts specific examinations of sustainable institutional design approaches for healthcare and long-term care that directly impact elderly well-being. We participate in two major initiatives: the National Bureau of Economic Research (NBER) project in the United States and the “MIRAI2.0” project involving 19 universities from Japan and Switzerland. Through these collaborations, we conduct international comparative research targeting 11 high-income countries that will enter super-aged societies within the next decade.
A critical factor in examining institutional design is understanding the relationship between price and quantity policies and stakeholder incentives on both supply and demand sides. This research reconstructs large-scale administrative data from participating countries into comparable datasets, treating institutional and policy changes within countries, as well as differences between countries and regions, as natural experiments. We aim to clarify behavioral changes among elderly individuals and service providers in counterfactual scenarios.
Furthermore, we seek to elucidate the mechanisms between such behavioral changes and elderly well-being as well as healthcare and long-term care costs. Additionally, with implementation in policy-making and service delivery contexts in mind, we will develop applications and explore pathways to contribute to evidence-based policy discussions among diverse stakeholders.
