• Enrollment record is personal information and will not be given to third parties.
  • If the former student is alive, s/he should request one the following: Certificate of Graduation (Diploma), Certificate of Withdrawal.
  • If the former student is deceased, approval of the Personal Information Protection Committee is required. Due to the limited dates of the committee’s meetings, this may take up to 2 months.
  • Information cannot be released without an appropriate reason, or for purposes other than confirming enrollment.
  • Fees cannot be refunded, regardless of the outcome of the investigation.

For a former student who is deceased

Requests by a family member will require:

  1. A copy of applicant’s family register showing the applicant’s relationship with the former student in question.
  2. A copy of one of the following: applicant’s driver’s license, health insurance card or passport.
  3. Fee of 1,000 yen per certificate. (Please send the money via Japan Post money order or International Reply-Paid Coupon)
  4. A stamped, self-addressed return envelope.
  5. Complete the following reqest form.

Fill in the application in detail. If many details are unknown, please contact us via email or fax prior to submitting the request and payment.

Requests by a non-family member will require:

  1. Letter of proxy from a family member.
