Documents Required at Hospital and Pharmacy Reception for Insurance-Covered Treatment

To All Patients Visiting the Health Support Center

The validity periods of current health insurance certificates/cards are expiring in sequence, with all cards ultimately expiring on December 1, 2025. To receive insurance-covered medical treatment going forward, you must present either your “Health Insurance Eligibility Certificate(資格確認書)” or “My Number Card” at hospital or pharmacy reception.

*If no valid document is presented, you will be required to pay the full cost of your visit, as has been the case previously.
*When visiting the Health Support Center, students must also present their student ID, and faculty/staff must also present their faculty/staff ID.

If you have questions about your Health Insurance Eligibility Certificate, contact your insurance provider (for example, your city or town office if you have National Health Insurance).
