◆Certificate of Health Check-up in English cannot be obtained through the Certificate Issuance Service (online application).

◆If you wish to obtain a Certificate of Health Check-up in English, it can be issued at the Health-care Room, Nishiwaseda branch or Tokorozawa branch of Health Support Center. 

By On-Campus certificate-issuing machine (Only in the university prescribed form in Japanese)

【Period】 From noon on Monday, June 1, 2026 to Wednesday, March 31, 2027
【Fee】 Certificate in Japanese: 300 yen / 1 copy

• To use the certificate issuing machines, you will need the “Print Reservation Number” which
  will be sent to you after applying online via the Certificate Issuance Service
  (Online Application).
  You cannot use your student ID card to print your certificate.
• The Certificate in English is not available at the On-Campus issuing machine. See the next section.
• If you cannot issue the certificate at the On-Campus issuing machine, please contact the Health-Care Room.
• Please refer to the following Waseda Portal Office HP for operating hours

Issuance at Health Support Center (University prescribed form in Japanese/English, Specified format)

Reservations are required. Reservations start from April 22. Please call the location you wish to visit to make a reservation.

※Please check health support center HP for temporary closure.

Waseda Campus : From Friday April 24, 2026 to Wednesday, March 31, 2027
Nisiwaseda Campus : From Thursday, April 23, 2026 to Wednesday, March 31, 2027
※Nishiwaseda Branch will not accept same-day reservations on April 23.
Tokorozawa Campus : From Thursday April 23, 2026 to Wednesday, March 31, 2027
※If you need the certificate after you are graduated or completed your program and returned your student ID to the university, come to any of the above offices with your ID (passport, residence card, etc.) on or before the last day of the month you are graduated or completed.

• If you need to take additional tests other than the student health check-up items, you may apply for the test at the Health-Care Room at your own expense, and obtain a medical certificate that includes the result of the additional test, there. Depending on the test item, dietary restrictions may be required. Please also note that it may take several days before the test result is out. Contact us well in advance.

Certificates can only be issued to currently enrolled students who have taken the fee-based health check-up for non-degree students during the prescribed period.
Please contact the Health-Care Room if you wish to have a certificate issued.
