If you wish to request an academic background verification, please send the following required documents to the email address below.

Required Documents:

–A valid official identification of the individual (e.g., copy of passport, residence card, My Number Card [front side only], Japanese driver’s license)
If submitting a My Number Card, please provide only the front side.Do not submit the back side displaying the personal identification number, as handling of such numbers is strictly regulated under Japanese law.
–A certificate issued by Waseda University for the individual
–A consent form (free format) clearly indicating the individual’s agreement to the academic background verification
–If the individual’s name has changed since their enrollment, an official document verifying the name change (e.g., copy of family register)
