{"id":35508,"date":"2026-07-23T14:24:29","date_gmt":"2026-07-23T05:24:29","guid":{"rendered":"https:\/\/www.waseda.jp\/fsci\/gips\/?p=35508"},"modified":"2026-07-23T14:24:29","modified_gmt":"2026-07-23T05:24:29","slug":"%e4%bd%93%e8%82%b2%e6%96%bd%e8%a8%ad%e5%88%a9%e7%94%a8%e7%94%b3%e8%ab%8b%e6%9b%b8%e3%81%ab%e3%81%a4%e3%81%84%e3%81%a6-2","status":"publish","type":"post","link":"https:\/\/www.waseda.jp\/fsci\/gips\/news\/2026\/07\/23\/35508\/","title":{"rendered":"Sports Facility Use Application Form"},"content":{"rendered":"<p class=\"isSelectedEnd\">The sports facilities (Gymnasium, Athletic Field, and Tennis Courts) can be used in the following two ways:<\/p>\n<ul data-spread=\"false\">\n<li><strong>Regular Reservation<\/strong><\/li>\n<li><strong>Same-Day Use (No Reservation Required)<\/strong><\/li>\n<\/ul>\n<h2><span style=\"background-color: #bfffff;\">Regular Reservation<\/span><\/h2>\n<p class=\"isSelectedEnd\">The sports facilities (Gymnasium, Athletic Field, and Tennis Courts) are available for regular use by submitting an application for each of the four periods of the academic year.<\/p>\n<p class=\"isSelectedEnd\">If you wish to continue using the facilities beyond one period, you must submit a new application for each subsequent period.<\/p>\n<div class=\"table-wrapper\"><table>\n<tbody>\n<tr>\n<th>Period<\/th>\n<th>Available Months<\/th>\n<\/tr>\n<tr>\n<td>Period 1<\/td>\n<td>April \u2013 July<\/td>\n<\/tr>\n<tr>\n<td>Period 2<\/td>\n<td>August \u2013 September<\/td>\n<\/tr>\n<tr>\n<td>Period 3<\/td>\n<td>October \u2013 December<\/td>\n<\/tr>\n<tr>\n<td>Period 4<\/td>\n<td>January \u2013 March<\/td>\n<\/tr>\n<\/tbody>\n<\/table><\/div>\n<h3>Application Procedure<\/h3>\n<ol start=\"1\" data-spread=\"false\">\n<li>Download the <strong>Regular Sports Facility Use Application Form<\/strong> and the <strong>Participant List<\/strong>.<\/li>\n<li>Complete both forms <strong>in Japanese<\/strong>, following the sample provided.<\/li>\n<li>Submit the completed forms by email to the <strong>IPS Office<\/strong> by the application deadline for the relevant period.<\/li>\n<\/ol>\n<p>For detailed information, please check\u00a0<a href=\"https:\/\/www.waseda.jp\/fsci\/gips\/en\/campuslife\/certificate\/\">IPS square<\/a><\/p>\n<h3>Reservation Results<\/h3>\n<p>\u30fbIf your reservation is approved<br \/>\nIf your requested date and time are available, a confirmation email will be sent to your Waseda email address.<br \/>\nAfter receiving the confirmation, please submit the <strong>Sports Facility Use Application Form<\/strong> to the IPS Office by email.<\/p>\n<p>\u30fbIf your reservation is NOT approved<br \/>\nIf your requested reservation cannot be accommodated, the IPS Office will notify you by email.<br \/>\nPlease select another date or time and submit a new reservation request.<\/p>\n<h3>Important Notes<\/h3>\n<ul data-spread=\"false\">\n<li>The reservation must be submitted by the <strong>team representative<\/strong>.<\/li>\n<li>Application forms must be completed <strong>in Japanese only<\/strong>.<\/li>\n<li>When using the Gymnasium, Athletic Field, or Tennis Courts, you must present your <strong>Facility Use Permit<\/strong> to the gymnasium attendant before using the facility.\n<ul data-spread=\"false\">\n<li><em>The permit will be issued at the Gymnasium Management Office when you check in for your first use of each month.<\/em><\/li>\n<\/ul>\n<\/li>\n<li>Please use the facility only during your reserved time.<\/li>\n<li>If you are unable to use the facility on your reserved date, you must cancel your reservation in advance.<\/li>\n<\/ul>\n<h2><span style=\"background-color: #bfffff;\">Same-Day Use Without a Reservation<\/span><\/h2>\n<p class=\"isSelectedEnd\">If you wish to use a sports facility without a reservation, you may do so <strong>only on the day of use<\/strong>, provided that the facility is available. Please present your <strong>Campus Card<\/strong> at the Gymnasium Management Office before using the facility.<\/p>\n<p class=\"isSelectedEnd\"><em>Submission of the <\/em><strong><em>Sports Facility Use Application Form<\/em><\/strong><em> and the <\/em><strong><em>Participant List<\/em><\/strong><em> is not required for same-day use.<\/em><\/p>\n<h3>Important Notes<\/h3>\n<p>The Gymnasium, Athletic Field, and Tennis Courts are <strong>reservation-based facilities<\/strong>. Even if a facility appears to be vacant, you may <strong>not<\/strong> use it without permission.<br \/>\nSame-day use is permitted only if the facility is available and <strong>after receiving approval from the gymnasium attendant<\/strong>.<br \/>\nDo not decide on your own to use the facility without authorization.<br \/>\nThese are shared public facilities.
