Notes

1. For application documents to be completed by applicants, please complete them in the language that applicants choose on the application forms page 1.
2. For the applicant evaluation form(s) and all required certifications, please complete them in Japanese or English. Otherwise, certified translation must accompany.
3. Documents will not be returned once submitted, regardless of any special circumstances on the part of the applicant. Original certificate which cannot be reissued may be returned only when a written request by the designated format, "Request Form for Return of Submitted Materials" must be submitted together with its photocopy at the time of application.
4. Contents of application documents once submitted cannot be changed.
5. If there is any falsification found in any of the documents presented for application, admission will be revoked. (We may contact the institution(s) that issued the certificates.) Application fee will not be refunded in this case.
