11. Statement of source of funds (designated form)

Not applicable to Special Permanent Resident, Permanent Resident, Long Term Resident, Spouse or Child of Japanese Citizen, Spouse or Child of Permanent Resident in Japan.
Please indicate the source and amount of funding for studying at GSAPS (usually 3 years for the PhD Program). Scholarship recipients must submit a scholarship award letter.

12. Certificate on Registered Matters in the Alien Registration

Applicable to applicants under Domestic Application Method. The certificate should state the status of residence and the authorized period of stay. The certificate should be obtained at a city / ward office. A photocopy of the Alien Registration card is not acceptable. Applicants who are in the process of extending their authorized period of stay are requested to submit a document to certify that they are in the process of the extension procedure.

13. Application for Certificate of Eligibility, etc. (designated form)

Applicants under the Foreign Application Method who wish to obtain a certificate of eligibility must enclose this application. Refer to section, "Student Visa".

14. Photocopy of the passport (page with photo)
