Please submit the following documents.

■For student at 2nd grade and above, and For Students who enrolled in September 2014.

B. [Optional] List of Previous Publications and Research(Please come and get the original copy of it which will be reused every semester).
■For students who enrolled in April 2015.

B. [Mandatory] List of Previous Publications and Research (Please fill out your name, student ID and other information in the form even if you do not have any publication and research to report yet).
